What Is Group Adult Foster Care?

Group Adult Foster Care (GAFC) is a MassHealth program that helps people (members) who need support with everyday tasks remain as independent as possible.

Through the program, members receive help from a direct care aide who visits the home on a regular schedule but does not live with them. Members also receive regular support from a nurse and care manager who help monitor their well-being and coordinate their care.

GAFC may be a good fit for people who need regular help with daily activities but do not need someone living with them full time.

Who Can Benefit from GAFC?

Group adult foster care may be appropriate for people who:

  • Have MassHealth coverage that includes GAFC services
  • Need help with tasks and routines
  • Want to continue living independently at home
  • Do not require someone to live with them

What Does a Direct Care Aide Do?

A direct care aide provides hands-on support during scheduled visits.

Depending on the member's needs, a direct care aide may help with:

  • Bathing and grooming
  • Dressing
  • Mobility and transfers
  • Meal preparation
  • Medication reminders
  • Household routines
  • Safety supervision
  • Light assistance with daily tasks

The direct care aide follows an established care plan and helps members remain safe, comfortable, and supported in their daily lives.

How Is GAFC Different from AFC?

The main difference is that a direct care aide does not live with the member in the GAFC program. Instead, they visit to provide support. In Adult Foster Care (AFC), the person who provides care lives in the same home as the member and helps with their routine and needs.
