Become an Adult Foster Care Caregiver

Who Qualifies to Become an AFC Caregiver?
To qualify as an AFC caregiver, you must:
- Be at least 18 years old
- Live with, or plan to live with, the member in a qualifying home
- Be able to safely provide the required ADL/IADL assistance and supervision
- Complete required background and exclusion checks
- Provide required health documentation
- Complete all required documentation
- Meet all caregiver, home, relationship, and program requirements
Some adult relatives may qualify. However, the member’s spouse, a parent of a minor member, and another legally responsible relative are excluded. Fortcom Health Services will review the proposed relationship and other program requirements during intake.
What Are an AFC Caregiver's Daily Responsibilities?
AFC caregivers provide daily care-plan responsibilities that may include hands-on assistance, cueing, supervision, ADL/IADL support, safety monitoring, recordkeeping, and reporting changes to the Fortcom Health Services care team. They do not provide skilled nursing services.
How Is the AFC Caregiver Stipend Structured?
An approved AFC caregiver may receive a monthly caregiver stipend. Payment begins only after program approval and depends on the member’s authorized service level and applicable requirements. Fortcom does not provide tax advice.
