At Fortcom Health Services, our team works together to support members, caregivers, families, physicians, and referral partners with compassion, professionalism, and attention to detail.

Who You May Work With

Your Fortcom Health Services team depends on the program and individual plan of care.You may work with the following professionals and program roles:

AFC Caregivers

AFC caregivers live with the member and provide daily assistance, cueing, supervision, ADL/IADL support, and safety support in an approved home setting. They do not provide skilled nursing services.

GAFC Direct Care Aides

GAFC direct care aides do not live with the member. Instead, they visit the member's home to provide approved personal care and help with activities of daily living, following the individualized care plan.

AFC/GAFC Nurses and Care Managers

Our nurses and care managers don't live with the member. They provide nursing oversight, care management, training, and ongoing support for AFC and GAFC members and caregivers.

Home Health Nurses

Home health nurses don't live with the patient, but visit the home to provide targeted nursing assessments, procedures, or treatments.

Home Health Aides

Home health aides don't live with the patient. They provide health-related personal care in the home, including bathing, dressing, grooming, toileting, and mobility, under professional supervision.

Continuous Skilled Nursing RNs/LPNs

Our RNs and LPNs provide more than two continuous hours per day of medically necessary nursing care. They don't live in the member's home.

Program and Administrative Staff

‍Our program and administrative staff provide training and ongoing program support behind the scenes.
