Adult foster care (AFC) is a MassHealth program for members who need help with everyday tasks and live with a caregiver in the same home. The caregiver helps with daily routines like bathing, dressing, meals, moving around, and other personal needs.

Members also receive nurse and care manager oversight to help make sure care stays safe and consistent.

Are You Caring for Someone But Don’t Live with Them?

A direct care aide visits the person’s home regularly to help with personal care, everyday tasks, and getting around safely. The member also gets support from a nurse and care manager who check in on their health and help coordinate care.

Unlike adult foster care, GAFC doesn’t have a caregiver living in the home. All help is provided through scheduled visits based on the member’s needs.

Need Short-Term Medical Care at Home?

Home health care provides short-term medical care at home for people recovering from illness, injury, surgery, or a recent hospital stay. Licensed nurses and therapists come to the home to provide treatment and support recovery based on a doctor’s care plan.  

This type of care is temporary and continues only while medical recovery support is needed.

Living with a Serious Health Condition?

Critical skilled nursing is for members who are not just recovering from an illness or hospital stay, but who need ongoing help managing serious health conditions.

Nurses come to the home to provide treatments, manage medical needs, and keep a close eye on the person's health. This level of care helps people receive the medical support they need while remaining at home.
