Are You Living with and Caring for Someone?
Adult Foster Care (AFC)
AFC is a MassHealth program for members who need help with everyday tasks and live with a caregiver in the same home. The caregiver helps with daily routines like bathing, dressing, meals, moving around, and other personal needs.
Members also receive nurse and care manager oversight to help make sure care stays safe and consistent.
Are You Caring for Someone But Don’t Live with Them?
Group Adult Foster Care (GAFC)
Group Adult Foster Care may provide daily assistance from a direct care aide who does not live with the member, together with nursing oversight and care management.
Eligibility, authorization, and direct care aide availability vary by area and current staffing.
Need Short-Term Medical Care at Home?
Home Health Care Services
Medically necessary nursing and home health aide services provided at home under an individualized plan of care.
Services may support acute, chronic, stable, terminal, rehabilitative, or longer-term needs, depending on thepatient’s needs, coverage, and authorization.
Living with a Serious Health Condition?
Continuous Skilled Nursing
Extended nursing care provided by licensed RNs or LPNs for children and adults who require more than two continuous hours of medically necessary nursing services per day.
Approved hours depend on the plan of care and payer authorization.
