At Fortcom Health Services, we help people throughout Eastern Massachusetts receive the care and support they need while remaining at home.

We provide a range of home-based services for individuals with daily support needs, recovery needs, and ongoing medical needs. Whether someone needs help with everyday activities, short-term care after an illness or hospital stay, or ongoing nursing support, our team is committed to helping them receive care in a safe, familiar environment.

Fortcom Health Services is an approved MassHealth contracted provider and follows applicable MassHealth, federal, state, local, and accrediting body standards.

Our Adult Foster Care Program has earned NCQA Case Management Accreditation for Long-Term Services and Supports. We have also received CHAP Full Accreditation for Home Health, reflecting our commitment to nationally recognized standards for quality and safety.
