Please complete the referral form below and a member of our team will follow up to learn more about the person’s needs, answer questions, and help you understand which services may be appropriate.
Referrals may be made for:
- Adult Foster Care (AFC)
- Group Adult Foster Care (GAFC)
- Home Health Care Services
- Intermittent Skilled Nursing
- Home Health Aide Services
- Continuous Skilled Nursing (CSN)
- Not Sure/Please Help Me Choose
Fortcom Health Services can review the referral, discuss possible service options, and explain the next steps. Eligibility, medical necessity, coverage, authorization, approved services, and start dates are determined by the applicable program or health plan.
