Adult Foster Care (AFC)
Caregivers who live in the same home provide daily support for people who need help with their routines.

A MassHealth program where a direct care aide visits regularly to help with everyday tasks, without living in the home.

Home Health Care Services
Medically necessary nursing and home health aide services provided at home under an individualized plan of care. Services may support acute, chronic, stable, terminal, rehabilitative, or longer-term needs, depending on the patient’s needs, coverage, and authorization.

Continuous Skilled Nursing (CSN)
Extended nursing care provided by licensed RNs or LPNs for children and adults who require more than two continuous hours of medically necessary nursing services per day. Approved hours depend on the plan of care and payer authorization.

Fortcom Health Services provides coordinated support through Adult Foster Care, Group Adult Foster Care, Home Health Care Services, and Continuous Skilled Nursing. Our team works with members, patients, caregivers, families, and referral partners to support respectful care at home.
